Description
Thyroxine-binding globulin (TBG) is a protein produced by the liver that binds to thyroid hormones in the bloodstream, primarily thyroxine (T4) and triiodothyronine (T3). Thyroid hormones are essential for regulating metabolism, growth, and energy production throughout the body.
TBG serves as a carrier protein for thyroid hormones, helping to transport them through the bloodstream and regulate their distribution to various tissues and organs. The majority of circulating thyroid hormones are bound to TBG, with only a small fraction remaining unbound (free) and biologically active.
Testing for TBG levels can provide valuable information about thyroid hormone metabolism and transport. Changes in TBG levels can affect the total concentration of thyroid hormones in the bloodstream, even if the levels of free (unbound) hormones remain within the normal range.
Conditions that can affect TBG levels include:
- Genetic variations: Inherited mutations in the TBG gene can lead to alterations in TBG production or function, resulting in either elevated or reduced TBG levels.
- Hormonal changes: Pregnancy, estrogen therapy, and certain hormonal contraceptives can increase TBG levels, leading to higher total thyroid hormone levels in the blood.

- Medications: Some medications, including corticosteroids and androgens, can influence TBG levels.
- Thyroid disorders: Thyroid hormone levels can indirectly affect TBG production, with increased thyroid hormone levels typically leading to decreased TBG levels and vice versa.
Measurement of TBG levels is typically performed as part of thyroid function tests, which also include assessments of thyroid-stimulating hormone (TSH), free thyroxine (FT4), and free triiodothyronine (FT3) levels. These tests help evaluate thyroid function and diagnose thyroid disorders such as hypothyroidism and hyperthyroidism.
