The SCL-70 antibody, also known as anti-topoisomerase I antibody, is a specific autoantibody associated with systemic sclerosis (SSc), also known as scleroderma. Systemic sclerosis is a chronic autoimmune disease characterized by fibrosis of the skin and internal organs, as well as vascular abnormalities.

The SCL-70 antibody targets topoisomerase I, an enzyme involved in DNA replication and repair. The presence of SCL-70 antibodies in the blood is highly specific for a subset of systemic sclerosis known as diffuse cutaneous systemic sclerosis (dcSSc), which is characterized by widespread skin involvement and internal organ fibrosis.

Testing for SCL-70 antibodies is typically performed using enzyme-linked immunosorbent assay (ELISA) or immunoblotting techniques. The presence of SCL-70 antibodies can aid in the diagnosis of systemic sclerosis, particularly in distinguishing between different subsets of the disease and predicting certain clinical manifestations, such as pulmonary fibrosis and renal involvement. However, not all individuals with systemic sclerosis will test positive for SCL-70 antibodies, and conversely, some individuals without systemic sclerosis may have positive SCL-70 antibody tests. Therefore, diagnosis should be based on a combination of clinical findings, laboratory tests, and sometimes imaging studies or biopsies.
